{"id":3073,"date":"2025-10-16T13:09:54","date_gmt":"2025-10-16T13:09:54","guid":{"rendered":"https:\/\/hfhpulm.com\/info\/?page_id=3073"},"modified":"2025-10-16T14:14:00","modified_gmt":"2025-10-16T14:14:00","slug":"cooling-to-help-injury-lungs-chill","status":"publish","type":"page","link":"https:\/\/hfhpulm.com\/info\/?page_id=3073","title":{"rendered":"Cooling to help injury lungs (CHILL)"},"content":{"rendered":"\n<p class=\"wp-block-paragraph\">Phase IIB Randomized Control Trial of Therapeutic Hypothermia (TH) plus Neuromuscular Blockade (NMB) in Patients with ARDS.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">For screening, contact via Halo: HFH DEM Research<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong><span style=\"text-decoration: underline;\">Inclusion Criteria: <\/span><\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Mechanically ventilated for \u2264 7 days (trach ok also)<\/li>\n\n\n\n<li>Severe ARDS by Berlin criteria: \n<ul class=\"wp-block-list\">\n<li>P\/F ratio &lt;200 with PEEP \u22658 cm H2O <\/li>\n\n\n\n<li>Bilateral infiltrates not completely explained by hydrostatic pulmonary edema or atelectasis <\/li>\n\n\n\n<li>Occurring within 7 days of exposure to a predisposing condition (ongoing exposure, not just initiation of the predisposing condition).<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li>Randomization must occur <strong>within 72 hours of ARDS<\/strong>.<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\"><strong><span style=\"text-decoration: underline;\">Exclusion Criteria:<\/span><\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Refractory hypotension (&gt;2 \u00b5g\/kg\/min norepi or equivalent for &gt;6h prior to randomization<\/li>\n\n\n\n<li>Core temp &lt; 35.5\u00b0C except CRRT for \u22656h on day of randomization<\/li>\n\n\n\n<li>Bleeding (&gt;3u blood products in &lt;24h, surgery or IR day on day of randomization<\/li>\n\n\n\n<li>Platelets &lt; 10K\/mm 3 day of randomization<\/li>\n\n\n\n<li>Active hematologic malignancy<\/li>\n\n\n\n<li>Skin process that precludes cooling<\/li>\n\n\n\n<li>Moribund (&lt;72 h predicted survival)<\/li>\n\n\n\n<li>Pre-morbid condition \u2013 unlikely 28d survival<\/li>\n\n\n\n<li>DNR (ie no CPR in the event of cardiac arrest)<\/li>\n\n\n\n<li>Unlikely to remain intubated for \u226548h<\/li>\n\n\n\n<li>Primary provider unwilling to participate<\/li>\n\n\n\n<li>Severe underlying lung disease (home O2, lung transplant; home vent (except OSA)<\/li>\n\n\n\n<li>BMI &gt; 50 kg\/m2<\/li>\n\n\n\n<li>NYHA class IV heart disease<\/li>\n\n\n\n<li>Acute coronary syndrome within 30d<\/li>\n\n\n\n<li>Cardiac arrest within 30d<\/li>\n\n\n\n<li>Burns &gt; 20% body surface<\/li>\n\n\n\n<li>Severe chronic liver disease (C-P 12-15)<\/li>\n\n\n\n<li>Previously randomized in CHILL trial<\/li>\n\n\n\n<li>Enrolled in another intervention study<\/li>\n\n\n\n<li>On ECMO<\/li>\n\n\n\n<li>Missed ARDS window (72h)<\/li>\n\n\n\n<li>Missed NMB window (48h)<\/li>\n\n\n\n<li>Missed MV window (7d)<\/li>\n<\/ul>\n","protected":false},"excerpt":{"rendered":"<p>Phase IIB Randomized Control Trial of Therapeutic Hypothermia (TH) plus Neuromuscular Blockade (NMB) in Patients with ARDS.
