ILD Outpatient referrals Workflow<\/u><\/strong><\/p>\n

For APPs and Pulmonary staff : <\/u><\/p>\n

    \n
  1. Let your patient know<\/strong> that you are reviewing with our team and that they will be called to schedule with the ILD team or hear from you with next steps.<\/li>\n
  2. Enter a consult order<\/strong> request for pulmonary \u00e0  interstitial lung disease.  Enter whatever details you would have emailed. <\/li>\n
  3. Order is automatically added to the consult queue (includes referring provider\u2019s name).<\/li>\n
  4. Clinic staff  routes to the ILD outpatient rounder for review.<\/li>\n
  5. ILD rounder enters documentation in the chart and responds to the staff message, cc\u2019s referring provider.<\/li>\n
  6. Clinic staff coordinates scheduling.<\/li>\n<\/ol>\n

     <\/u><\/p>\n

    For fellows<\/u>: please find the outpatient ILD rounder to review as we may simply try to staff with you.<\/p>\n

    <\/p>\n

    The process would be the same for sarcoid (select \u2018sarcoid\u2019 on the consult request).  This process is already in place for pulmonary hypertension consults.<\/p>\n

     <\/u><\/p>\n

    Other appointments<\/u><\/strong><\/p>\n

    Inpatient needing an outpatient consult (new to ILD team)?<\/u><\/p>\n

    Follow the same process as a new outpatient referral.   <\/p>\n

    Consider discussion with inpatient ILD team to check for any pre-discharge needs.<\/p>\n

     <\/u><\/p>\n

    Established ILD patient needing followup?<\/u>  We appreciate the already excellent communication from HFH, WBH and Macomb and predict an increase in the number of our patients admitted to Wyandotte. <\/p>\n

    Please have your team Halo the ILD inpatient team prior to discharge to arrange followup.<\/p>\n

    Requesting ILD Multidisciplinary Conference Review only (not a consult)? <\/u><\/strong><\/p>\n

    Email the ILD outpatient rounder(listed in Momentum as \u2018ILD OPD\u2019) with the information listed below (tumor board-like approach).  Let us know if you will be attending.<\/p>\n

    Fellows, please cc your staff for the patient.<\/p>\n

    Occupational history\/Exposures<\/strong>?: any relevant\/unknown
