Effective July 10, 2019 there will be changes to the existing approved outpatient and inpatient attestation statements. The most significant change will be the removal of the drop down menu that indicates the type of provider that you are supervising. This drop down menu will be replaced with “resident/fellow”. As a result, these new statements cannot be used for APP services. For APP services, you must use the existing approved APP attestation statements:
.CLINIC APP SHARED VISIT – I saw and evaluated the patient with the APP and agree with their findings and plan. I personally participated in the {HFHS EXAM/HISTORY/DECISION:24446} and my findings were ***. @ME@
.IP APP SHARED VISIT – I saw and evaluated the patient on the date of the APP’s note and I agree with their findings and plan. I personally participated in the {HFHS EXAM/HISTORY/DECISION:24446} and my findings were ***. @ME@
