ILD or Sarcoid Outpatient Consults & Requests for Multidisciplinary Conference Review

ILD Outpatient referrals Workflow

For APPs and Pulmonary staff :

  1. Let your patient know that you are reviewing with our team and that they will be called to schedule with the ILD team or hear from you with next steps.
  2. Enter a consult order request for pulmonary à  interstitial lung disease.  Enter whatever details you would have emailed. 
  3. Order is automatically added to the consult queue (includes referring provider’s name).
  4. Clinic staff  routes to the ILD outpatient rounder for review.
  5. ILD rounder enters documentation in the chart and responds to the staff message, cc’s referring provider.
  6. Clinic staff coordinates scheduling.

 

For fellows: please find the outpatient ILD rounder to review as we may simply try to staff with you.

The process would be the same for sarcoid (select ‘sarcoid’ on the consult request).  This process is already in place for pulmonary hypertension consults.

 

Other appointments

Inpatient needing an outpatient consult (new to ILD team)?

Follow the same process as a new outpatient referral.   

Consider discussion with inpatient ILD team to check for any pre-discharge needs.

 

Established ILD patient needing followup?  We appreciate the already excellent communication from HFH, WBH and Macomb and predict an increase in the number of our patients admitted to Wyandotte. 

Please have your team Halo the ILD inpatient team prior to discharge to arrange followup.

 

Requesting ILD Multidisciplinary Conference Review only (not a consult)?

Email the ILD outpatient rounder(listed in Momentum as ‘ILD OPD’) with the information listed below (tumor board-like approach).  Let us know if you will be attending.

Fellows, please cc your staff for the patient.

Smoking status:  never/former/current,
Occupational history/Exposures?: any relevant/unknown
Autoimmune sign/symptoms/serology?: any relevant (we can review what labs were ordered during conference)
