Pathology consults

External pathology for review:

1. Order ‘surgical specimen exam

2. In the history, include the date of the biopsy and “Attn: ___” if you need a specific pathologist to read the slides (contact the pathologist directly to let them know there is path you want to review with him/her).   You can also include a ‘due by’ date if this is for a conference so that the slides get to the pathologist in a specific timeframe.

3. After signing the order, click on the order and click on ‘reprint requisition’ to print this.

3. Drop off the pathology slides with the outside report  on E&R 6 in the pathology receiving room.

4. Fill in the patient and your contact info in the ‘Outside pathology chain of custody’ binder

5. Leave path in the bin marked – ‘OSS’ (there is a ‘stat’ and routine blue bin on the shelf)).

6. The slides are returned by the pathology department to the institution of origin.

Sending out HFHS pathology to another facility

1. Order ‘surgical specimen exam’

2. In the history include the date and accession number and type of biopsy , brief history, and institution (and specific pathologist if relevant) to whom you are sending.

3. Include billing information (patient’s insurance or cost center number for pulmonary.  Pathology cost center only pays for this if our pathologist is the one requesting the second opinion).

4.Email PathFileRm@hfhs.org the MRN, original accession #,  date of biopsy and where you want it sent.  You can also call (313) 916 3953  (someone covers the phone in the fileroom from 6am-2:30 pm)
