Please note, it will also help us if you indicate in the comments what ‘other’ is for (like MAB start). Additionally, this can be used for patients seen in the satellite clinics — however, if the patient needs to be seen face-to-face they will have to come to Henry Ford Main.
You can use this for VAIM testing, medication access, medication education, etc.
