In an effort to improve the quality of care across all of our ICU pods, we have been performing a trial of multi-disciplinary “Quality Rounds.” Based upon the most recent feedback from all the caregiver disciplines, efficiency of quality rounds has been a concern.
At the most recent ICU collaborative meeting, we decided to modify the Quality Rounds so that they would occur in a single location in the morning, “huddle” style. Each caregiver domain will be in charge of bringing relevant information to the huddle. Eventually, large dry erase boards will be used to keep track of everything. For now, we can continue using the following form to track each domain: Quality Rounds List
– Mobility: Mobility goal for the day – use Level
– Nutrition: Diet info
– Nurse: Central lines/foley information
– Pharmacy: Chlorhex, DVT prophy, PUD prophy, sedation protocol
-RT: SBT success
– MD: Runs rounds; enters any of the above into EPIC for real time followthrough (medication or nursing order)
Rounding times. Schedule per pod:
MORNING: Make sure previous day’s list is available for reference.
– Pods 1-3: Starts in Pods 1-2 back hallway at 8am. Start with Pod 1 patients at 8AM, then Pod 3, then Pod 2. This order allows for Pod 3 nursing leader to be present between 815AM-8:20AM to complete all Pod 3 patients.
– Pod 4: Start at 8am.
– Pod 5: Start at 830 AM
AFTERNOON
-All pods start at 3pm. Fellow, nurse, and mobility. Goal is to ensure that follow through has occurred on all items on the rounding tool. To do list generated in AM can be used to do targetted followup to check for deficincies.
